What are North Bay Center Services?

North Bay provides Community Living Support (CLS) services for adults 18 and older with intellectual/developmental disabilities in the community.  Community Living Supports are services that increase or maintain personal self-sufficiency, assist in achieving the individual’s goals of community inclusion, participation, independence, or productivity.  CLS services assist individuals by prompting, guiding, teaching and providing hand over hand assistance to increase their independence in their home or the community.  Skill development is the primary focus of CLS services, with an emphasis on teaching individuals the skills necessary for participation in the community and at home. CLS services are intended to be time limited and are expected to gradually fade as the individual gain the necessary skills to function more independently.

Services are person-centered and driven by the individual’s unique needs, wants, preferences and desired outcomes. Individuals must meet medical necessity through the Person-Centered Planning process, the CLS Assessment process and be reviewed by the CLS Committee.  CLS goals and objectives are identified in the Individual Plan of Service which outlines the amount, scope, and duration of the services.  The need for CLS services will be reviewed at least annually during the Person-Centered Planning process, or more frequently as needed, to ensure services remain appropriate and aligned with the individual’s current needs and goals.

The goal of the North Bay Center is to provide support to individuals. We provide supports to people that help them reach their greatest potential in life through a variety of activities and experiences that are their choice, and are part of their dreams for the future.
